Dementia is hugely underdiagnosed and under-managed partly due to stretched specialist services. Written by a team involved in a pioneering community-based primary care service, this practical book brings together 99 case studies from the frontline in providing early intervention for people seeking help for suspected dementia. Both typical and atypical cases of Alzheimer's disease and other dementias are examined, familiarising readers with possible patient scenarios and the recommended management strategies. Useful tools such as example forms for history taking and the use of a modified General Practitioner Assessment of Cognition (GPCOG) equip multidisciplinary teams with the knowledge needed for early identification of dementia. The final chapter sets out key considerations in primary care such as carer support, sharing diagnoses, and forming a dementia-friendly community. Cestodes of the family Anoplocephalidae parasitize a wide range of usually herbivorous hosts including e.g. rodents, ungulates, primates, elephants and hyraxes. While in some hosts, the epidemiology of the infection is well studied, information is lacking in others. In this study of mountain gorillas in the Virunga Massif, an extensive sample set comprising adult cestodes collected via necropsies, proglottids shed in feces, and finally, fecal samples from both night nests and identified individuals were analysed. Anoplocephala gorillae was the dominant cestode species detected in night nest samples and individually known gorillas, of which only 1 individual hosted a Bertiella sp. It was shown that the 2 species can be distinguished through microscopy based on egg morphology and polymerase chain reaction (PCR) assays for diagnostics of both species were provided. Sequences of mitochondrial (cox 1) and nuclear (ITS1, 18S rDNA, 28S rDNA) markers were used to evaluate the phylogenetic position of the 2 cestodes detected in mountain gorillas. Both types of fecal samples, from night nests and from identified individuals, provided comparable information about the prevalence of anoplocephalid cestodes, although the analysis of samples collected from identified gorilla individuals showed significant intra-individual fluctuation of A. gorillae egg shedding within a short period. Therefore, multiple samples should be examined to obtain reliable data for wildlife health management programmes, especially when application of anthelmintic treatment is considered. However, while A. gorillae is apparently a common symbiont of mountain gorillas, it does not seem to impair the health of its host.

Background and Aims:
Atrial fibrillation (AF) is the most important cause of embolic stroke of undetermined source (ESUS). Implantable loop recorder (ILR) demonstrated the highest sensitivity for detecting it. This register was created to confirm the high prevalence of AF in patients after ESUS and to verify possible benefits on clinical outcomes such as TIA (Transient Ischaemic Attack)/stroke recurrence and death using ILR.
Methods:A total of 278 patients admitted to “Molinette” Hospital in Stroke Unit department between 2011 and 2016, diagnosed with ESUS, underwent ILR implantation if they had at least one risk factor for AF. A total of 165 patients admitted to other departments in the same center for the same pathology, without ILR, represent the control group. We used propensity score to select 132 patients from each group (matching age, sex, CHADS-VASC, and HAS-BLEED baseline characteristics).
Results:The detection rate of AF episodes was significantly higher in the ILR group (p < 0.001). No significant protective role of ILR for clinical endpoints was found on univariate analysis, although a trend towards significance has been pointed for the composite outcome of death and ischemic events recurrence (OR 0.52, CI 0.26–1.04, p = 0.06). A protective role of ILR was found for deaths (OR 0.4, CI 0.17–0.94, p 0.03) and for the composite outcome (OR 0.41, CI 0.19–0.87, p 0.02) on multivariate analysis in the best subsets.
Conclusion:With our statistical models, we identified a significant clinical benefit from ILR monitoring, evidenced by a trend of less death and TIA/stroke recurrence and relevant ILR protection for prediction of TIA/stroke recurrence.

Summary
In the aftermath of World War II, the United States almost singlehandedly created the web of international institutions and organizations that comprise what is often referred to as the “liberal world order.” In the economic realm, American policymakers, working most closely with the British, designed what would become the International Monetary Fund and the World Bank. On trade matters, the United States considered plans for an International Trade Organization, although it ultimately designed and backed the less ambitious General Agreement on Trade and Tariffs. These efforts put in place international regimes and organizations that remain consequential today. But by far the highest-level American attention was reserved for the United Nations (UN), a new global organization designed to ensure future world peace and security.

Background: Disinfection procedures are an essential aspect of preventing cross contamination, especially in situations where the risk of infection is higher, such as in operating rooms (ORs). Disinfection procedures in ORs at the end of each surgery session are not the same as final cleaning procedures. We assessed the difference in microbial contamination between different levels of disinfection, before T(0) and after T(1) the use of an ultraviolet C device (UVC-D). Methods: A cross-sectional study was conducted between December 2019 and August 2020 in a private clinic. Three sanitation levels (SL1–SL3) were compared for the reduction in colony-forming units (CFU) between T(0) and T(1): (1) no disinfection after surgery (SL1);, (2) after in-between cleaning (SL2), and (3) after terminal cleaning (SL3). UVC-D was used for 6 minutes, 3 minutes per bed side. Overall, 260 Petri dishes were used in 3 ORs, incubated at 36°C, and CFU were counted after 48 hours. Descriptive statistics, Wilcoxon test, and MANOVA for repeated measures were performed to verify the 95% statistical difference between T(0) and T(1), both on the whole sample and combined with the different SLs. Results: The unstratified analysis showed statistically significant differences (Wilcoxon test, p < 0.05) between T(0) and T(1), with means and standard deviations of 11.42 ± SD 41.19 CFU/PD and 5.91 ± SD 30.89, respectively. The Manova test for repeated measures, applied to 54 pairs of measurements, showed no significant difference between SLs in T(0)-T(1) CFU reduction. Overall, the mean percent reduction in CFU was 93.48% (CI95% = 86.97-99.99%). Conclusions: The results showed significant improvements in disinfection under any condition tested with UVC-D. Using the device immediately after surgery (SL1), before standard cleaning procedures, reduced CFUs by 97.3%. In some situations, UVC light was sufficient to reduce CFU to zero, even without chemical and mechanical cleaning. However, we do not recommend this approach; UVC light disinfection should be applied only after sanitization procedures because it does not remove dirt.

One of the unique challenges that the International Criminal Court's (ICC's) Office of the Prosecutor (OTP) faces is deciding when and where to launch investigations. It is a task that other international prosecutors have not confronted. Their investigative “situations” were selected in advance, leaving those prosecutors free to focus on the myriad other challenges any international justice enterprise faces. The ICC prosecutor's ability to define her own investigative situations (within the limits of jurisdiction) is both a boon and a burden. On the one hand, it accords the OTP the freedom to select the situations it deems most serious and worthy of international attention. Yet this discretion can also generate intense strain for the prosecutor of a still novel and fragile institution.

In November 2016, the Office of the Prosecutor (OTP) at the International Criminal Court (ICC) produced an extensive report documenting the status of ten “preliminary examinations” it is conducting. These examinations cover situations in which crimes under the jurisdiction of the Court may have been committed but the prosecutor has not yet decided to open a full investigation. The OTP's reporting practice has shed additional light on a process that has been opaque for much of the Court's existence and that has attracted relatively limited scholarly and specialist attention.
